  Sante Home Health is seeking a Clinical Supervisor-RN. This position is based in the Operations office, located in N. Scottsdale. The Clinical Supervisor assists with the supervision of home health services to patients in accordance with physician orders and strives to provide the highest quality of care. Skilled nursing and other therapeutic services furnished are under the supervision of the Clinical Supervisor.

Consults with the Director of Clinical Services and Administrator in determining a staffing pattern which will achieve patient care and promote a maximum level of utilization of health personnel.
Assists with the development, implementation and evaluation of patient plans of care.
Reviews patient medical records for compliance and conducts chart audits. 
Assists Director of Clinical Services in conducting patient care conferences, staff meetings and maintains documentation. Participates in community education projects.
Ensures that all necessary supplies and equipment are available.
Must be able to perform nursing functions in times of staff shortage.
Available to share on-call with nursing personnel as needed.
Ensures that documentation is completed in accordance with Agency policy.
Assists with staffing visit assignments and on-call.
Assists in the preparation and implementation of policies and procedures which meet Medicare, Medicaid, The Joint Commission, state and local laws.
Assists in the organization of nursing services to delineate and delegate functional responsibility, lines of relationship and communication to provide safe and therapeutically effective service.
Participates in the coordination of Agencys services with the services of other community agencies.
Develops a cooperative relationship and communicates effectively and professionally with physicians.
Investigates and reports any problem relating to patient care and/or employee well-being.
Assists in recruiting, hiring, evaluating and terminating nursing personnel.  Assists in instituting, interpreting and documenting disciplinary action.
Responsible for the orientation of new patient care personnel.


Nursing Degree from an accredited college or university. 
Prior to date of hire, must hold a current, valid RN license or temporary RN license through the Arizona State Board of Nursing, or hold a compact RN license. Must maintain valid licensure to practice as a RN. 
Minimum three (3) of experience in nursing,
Minimum one (1) year experience in home health nursing, strongly preferred.
Current CPR certification required. 
Must hold a valid drivers license with required state insurance.
