Sante of Chandler is seeking a day shift Certified Nursing Aide (CNA) with some experience in rehab or long term care. The Certified Nursing Aide (CNA) is responsible for providing each of his/her assigned guests with routine daily nursing care and services in accordance with the guests assessment and care plan, and as may be directed by the Director of Nursing and/or Nurse. Duties include but are not limited to:
Check each guest routinely to ensure that his/her personal care needs are being met in accordance with his/her wishes and the care plan.
Review care plans daily to determine if changes in the guests daily care routine have been
Inform the Admissions Nurse and/or Director of Nursing of any changes in the guests condition so that appropriate information can be entered on the guests care plan.
Assist guests with daily needs, including preparation for activity and social programs, and transporting guests to/from appointments.
Ensure that guests rooms are ready for receiving and make guests comfortable.
Assist guests during admissions, transfers and discharges.
Assist with turning, lifting, positioning and transporting guests into and out of beds, chairs, bathtubs, wheelchairs, lifts, etc.
Record all entries on flow sheets, notes, charts, etc., in an informative and descriptive manner.
Participate in and receive the nursing report upon reporting for duty.
