  The LPN (Licensed Practical Nurse) is responsible for providing direct nursing care to the patients, and to supervise the day-to-day nursing activities performed by nursing assistants. Such supervision must be in accordance with current federal, state, and local standards, guidelines, and regulations that govern our facility, and as may be required by the Director of Nursing to ensure that the highest degree of quality care is maintained at all times.


General Responsibilities:
  • Direct the day-to-day functions of the CNAs.  
  • Admit, transfer, and discharge guests.  
  • Enter physicians orders into guest electronic medical record.  
  • Electronically chart nurse notes in an informative and descriptive manner.  
  • Prepare and administer medications as ordered by the physician.  
  • Ensure that an adequate supply of floor stock medications, supplies, and equipment is on hand to meet the nursing needs of the guests.  
  • Requisition and arrange for diagnostic and therapeutic services, as ordered by the physician.  
  • Consult with the patients physician in providing the guest's care, treatment, rehabilitation, etc., as necessary.  
  • Administer professional services such as; catheterization, tube feedings, suction, applying and changing dressings/bandages, packs, colostomy, and drainage bags, taking blood, giving massages and range of motion exercises, care for the dead/dying, etc., as required.  
  • Take and record TPRs, blood pressures, etc., as necessary. Obtain sputum, urine and other specimens for lab tests as ordered.


Minimum Qualifications:
  • Must possess, as a minimum, a Nursing Degree from an accredited college or university, or be a graduate of an approved LPN/LVN program.  
  • Must currently hold, or be able to obtain, an unencumbered LPN/LVN license through the Arizona State Board of Nursing.  
  • Must have at least one (1) year experience working as a LPN.  
  • Recent experience in a LTC/SNF or transitional rehabilitation setting required.
