Sante of Surprise is seeking Licensed Practical Nurses (LPNs) for PT weekend night shifts

Shifts available:

Night Friday and Saturday
Night Saturday only

The Licensed Practical Nurses (LPN) is responsible for providing direct nursing care to the guests,  and to supervise the day-to-day nursing activities performed by nursing  assistants.

 ~Admit, transfer, and discharge guests as required.
 ~Perform administrative duties such as completing medical forms, reports, evaluations, studies,
 charting, etc., as necessary.
 ~Complete and scan required recordkeeping forms/charts upon the guests admission, transfer,
 and/or discharge.
 ~Encourage attending physicians to review treatment plans, record and sign their orders, progress
 notes, etc., in accordance with established policies.
 ~Receive telephone orders from physicians and record on the Physicians Order Form. Enter
 physicians orders into guest electronic medical record.

~Chart nurses notes in an informative and descriptive manner that reflects the care provided to the
 guest, as well as the guests response to the care.
 ~Report all discrepancies noted concerning physicians orders, diet change, charting error, etc., to
 the Director of Nursing.

~Ensure that appropriate documentation concerning unauthorized discharges is entered in the

 guests medical record in accordance with established procedures.
 ~Prepare and administer medications as ordered by the physician. Verify the identity of the guest
 before administering the medication/treatment. Ensure that prescribed medication for one guest is
 not administered to another.
 ~Order prescribed medications, supplies, and equipment as necessary, and in accordance with
 established policies.
 ~Ensure that narcotic records are accurate for your shift. Notify the Director of Nursing of all drug
 and narcotic discrepancies noted on your shift.
 ~Review medication cards for completeness of information, accuracy in the transcription of the
 physicians order, and adherence to stop order policies. Notify the attending physician of
 automatic stop orders prior to the last dosage being administered.

~Ensure that an adequate supply of floor stock medications, supplies, and equipment is on hand to
 meet the nursing needs of the guests. Report needs to the Director of Nursing.
 ~Receive/give the nursing report upon reporting in and ending shift duty hours

  • Must currently hold, or be able to obtain, an unencumbered LPN/LVN license through the Arizona State Board of Nursing.  
  • At least one (1) year experience working as a LPN in a LTC/SNF or transitional rehabilitation setting preferred.
  • A graduate of an accredited school of nursing
