Sante of Surprise is seeking to hire a talented, Occupational Therapist (OT). The Occupational Therapist (OT) will provide treatment related to Neurological Rehabilitation, Cardiac Recovery, and Orthopedic Care in a our state-of-the-art skilled nursing facility. The Occupational Therapist (OT) is responsible for providing and directing Occupational Therapy services to guests, including assessment, treatment, program planning and implementation, related documentation, and communication and functions under physicians orders.

  • Provides Occupational Therapy services to guests in a safe and effective manner.
  • Provides treatment in accordance with established standards of practice, and department procedures.
  • Conduct screenings of guests newly admitted to the facility and referred by nursing to determine need for intervention/treatment.
  • Evaluates guests to obtain data necessary for treatment planning and implementation.
  • Conducts specialized evaluations as indicated.
  • Interprets and communicates evaluation findings to guests, family, and other team members.
  • Establishes individual guest schedules in conjunction with other team members.
  • Develops treatment plans by identifying a problem list, including short and long term goals and methods to achieve identified goals.
  • Develops appropriate home or community programming to maintain and enhance the
     performance of the guest in their own environment.

Key Qualifications:

  • Bachelor, Masters, or Doctorate of Occupational Therapy from an accredited program.
  • Successful completion of National Certification Examination for Registered Occupational Therapist.
  • Licensure as an Occupational Therapist through the Arizona State Board of Occupational Therapy
