Sante of Mesa is seeking to hire Occupational Therapist/OT for PRN shifts and is conveniently located off U.S. 60 and Higley.
Our Occupational Therapists provide treatment related to Neurological Rehabilitation, Cardiac Recovery, and Orthopedic Care in a state-of-the-art, highly rated skilled nursing facility.
The Occupational Therapist is responsible for providing and directing Occupational Therapy services to guests, including assessment, treatment, program planning and implementation, related documentation, and communication and functions under physicians orders.
Provides treatment in accordance with established standards of practice, and department procedures.
Conduct screenings of guests newly admitted to the facility and referred by nursing to determine need for intervention/treatment.
Evaluates guests to obtain data necessary for treatment planning and implementation. Conducts specialized evaluations as indicated.
Interprets and communicates evaluation findings to guests, family, and other team members.
Establishes individual guest schedules in conjunction with other team members.
Develops treatment plans by identifying a problem list, including short and long term goals and methods to achieve identified goals.
Implements and conducts treatment as outlined in treatment plan.
Monitors guests response to intervention and modifies treatment as indicated to attain goals in accordance with physician orders.
Develops appropriate home or community programming to maintain and enhance the
performance of the guest in their own environment.
Documents results of guests assessment, treatment, follow-up, and termination of services.
Adapts treatment to meet the individual needs of the guest.
Provides/recommends/fabricates adaptive devices, orthotics or prosthetics. Trains guest, family, and appropriate staff in the use or application of the above.
Participates in discharge planning.
Prefer three (3) years of experience. Require a minimum of one year Occupational Therapy experience in an acute care hospital, facility or rehab setting.
Bachelor, Masters, or Doctorate of Occupational Therapy from an accredited program.
Successful completion of National Certification Examination for Registered Occupational
Current Arizona state licensure
