Sante Home Health is hiring OT's for PRN shifts in two areas of the valley, Central North and West Valley. Our Home Health OT provides and directs services to guests, including assessment, treatment, program planning and implementation, related documentation, communication and functions under physicians orders.

  • West Valley including Surprise, Glendale, Peoria, Goodyear, Litchfield Park, and Tolleson.
  • North - Central Valley including North Phoenix, Scottsdale, Cave Creek, Carefree, Anthem, and Fountain Hills.

Provides Occupational Therapy services to guests in a safe and effective manner. Provides treatment in accordance with established standards of practice, and department procedures.
Conduct screenings of guests newly admitted to the facility and referred by nursing to determine need for intervention/treatment.
Evaluates guests to obtain data necessary for treatment planning and implementation. Conducts
specialized evaluations as indicated.
Interprets and communicates evaluation findings to guests, family, and other team members.
Establishes individual guest schedules in conjunction with other team members.

  • Bachelor or Master of Occupational Therapy from an accredited program required; prefer Doctorate.
  • Successful completion of National Certification Examination for Registered Occupational Therapist.
  • Current Licensure as an Occupational Therapist through the Arizona State Board of Occupational Therapy.
  • Experience in home health, skilled nursing or assisted living preferred.
