  • Provides Physical Therapy services to guests in a safe and effective manner
  • Provides treatment in accordance with established standards of practice and department procedures
  • Evaluates guests to obtain data necessary for treatment planning and implementation. Conducts specialized evaluations as indicated.
  • Establishes individual guest schedules in conjunction with other team members.
  • Prior Physical Therapy/PT experience, in-patient experience in SNF/LTC/Rehab environment a plus!.
  • Credentialed Graduate of an accredited Physical Therapy program
  • Passing score on the National Physical Therapy Examination
  • Currently licensed as a Physical Therapist by the state of Arizona; Masters or Doctorates preferred.
  • Must have completed 2000 hours of direct supervision (Only applies to PRN positions)
