Sante Rehabilitative Services is seeking an Outpatient Physical Therapist (PT) for a full time position who will treat residents at our partner location, Holiday Retirement at Westgate Village in Glendale AZ.

The Physical Therapist (PT) is responsible for providing and directing outpatient Physical Therapy services to residents, including assessment, treatment, program planning and implementation, related documentation, and communication and functions under physicians orders. The Physical Therapist (PT) will evaluate and treat the residents with a variety of diagnoses and disabilities. The Therapist will focus on balance and fall prevention, therapeutic exercises for functional strengthening, patient/family education, mobility, core strengthening, and pain management.

Provides Physical Therapy services to residents in a safe and effective manner. Provides treatment in accordance with established standards of practice, and department procedures.
Conduct screenings of residents in the facility and referred by nursing to determine need for intervention/treatment
Evaluates residents to obtain data necessary for treatment planning and implementation. Conducts specialized evaluations as indicated.
Interprets and communicates evaluation findings to residents, family, and other team members.
Establishes individual guest schedules in conjunction with other team members.
Develops treatment plans by identifying a problem list, including short and long term goals and methods to achieve identified goals

  • Bachelor, Maters, or Doctorate of Physical Therapy from an accredited program.
  • Current licensure as an Physical Therapist through the Arizona State Board of Physical Therapy.
  • Minimum of two (2) years licensed experience required with Geriatric and Home Health experience strongly desired.
  • Successful completion of National Certification Examination for Registered Physical Therapist.
