Sant of Surprise is seeking to hire a Speech Language Pathologist (SLP) PRN. We are a 5-star Medicare rated property and had a Deficiency- free Arizona Department of Health Services annual survey.

The Speech Language Pathologist (SLP) is responsible for providing and directing Speech Therapy services to guests, including assessment, treatment, program planning and implementation, related documentation, and communication and functions under physicians orders.

Provides Speech Therapy services to guests in a safe and effective manner. Provides treatment in accordance with established standards of practice, and department procedures.

Conduct screenings of guests newly admitted to the facility and referred by nursing to determine need for intervention/treatment.

Evaluates guests to obtain data necessary for treatment planning and implementation.

Conducts specialized evaluations as indicated.

Interprets and communicates evaluation findings to guests, family, and other team members.

Establishes individual guest schedules in conjunction with other team members.

Develops treatment plans by identifying a problem list, including short and long term goals and methods to achieve identified goals.



  • Masters degree in Speech-Language Pathology
  • Licensure as a Speech Language Pathologist through the Arizona Department of Health Services.
  • Certificate of Clinical Competence from the American Speech Language Hearing Association.
